RN/LPN

RN/LPN Job Description

We are looking for a Registered Nurse to join our nursing staff in providing excellent health care to our patients. Your duty of caring for the patients coming into our facility includes tasks like creating and maintaining accurate patient records, administering medication and monitoring for side effects, recording vital signs, and reporting symptoms and changes in patient conditions.

A Bachelor’s degree in nursing and a state license are essential requirements for consideration. A professional, friendly attitude and effective communication skills are keys to becoming successful in this role.

Registered Nurse Responsibilities:

  • Maintaining accurate, complete health care records and reports.
  • Administering medications to patients and monitoring them for side effects and reactions.
  • Prescribing assistive medical devices and related treatments.
  • Recording patient vital signs and medical information.
  • Ordering medical diagnostic and clinical tests.
  • Monitoring, reporting, and recording symptoms or changes in patient conditions.
  • Administering non-intravenous medications.
  • Assessing, implementing, planning, or evaluating patient nursing care plans by working with healthcare team members.
  • Modifying patient health treatment plans as indicated by patient conditions and responses.

Registered Nurse Requirements:

  • Bachelor degree or master degree in nursing.
  • Current state licensure as a registered nurse and BLS certification required.
  • American Heart Association BLS/CPR
  • Ability to effectively communicate with patients, families, physicians, and hospital staff.
  • Basic computer skills.
  • Professional, friendly attitude.
  • Experience in-home health care.
  • Experience in the emergency room or intensive care.

HHA

HHA Job Description

We have an opening for a Home Health Aide to join our growing team. You will be tasked with helping patients with personal hygiene, minor medical needs, housekeeping, and other daily needs as required by the patient. To excel in this role you should have an interest in health care and the ability to work with limited supervision keeping the patient’s needs as your top priority.

Home Health Aide Responsibilities:

  • Help patients with personal hygiene, dressing, bathing and other daily tasks.
  • Perform basic health care services for patients including checking vital signs or administering prescription medication.
  • Help with general light housekeeping.
  • Make transportation arrangements as needed.
  • Work with a nurse, personal care aide, a CNA, nurse aide, nursing assistant, and other caregivers and in-home care professionals.
  • Observe and report on patient condition.
  • Keep up with in-service training.

Home Health Aide Requirements:

  • High school diploma.
  • Completion of 75 hours of training certificate (training provided)
  • Pass BCI/ FBI ( 5123.081)
  • Reliable transportation.
  • Must be at least 18 years old.
  • Must be able to pass a criminal background check.
  • Must be able to provide references.
  • Ability to work with limited supervision.
  • Ability to follow written and verbal instructions.
  • Have good personal hygiene.
  • Able to do light house keeping.

Direct Care Provider (DODD)

Direct Care Provider for DODD

We are looking for a dedicated Direct Care Worker to assist clients living with mobility limitations. Your duties will include performing housekeeping tasks, assisting clients with meals, and taking care of their personal hygiene needs. As a Direct Care Worker, your duties will also include running errands.

To ensure success as a Direct Care Worker, you should possess knowledge of best practices in providing care and experience in a similar role. Ultimately, an outstanding Direct Care Worker will be someone who can be entrusted with the wellbeing of clients and one who demonstrates a caring approach.

Direct Care Worker Responsibilities:

  • Providing immobile, ill, or frail clients with assistance, company, and comfort.
  • Administering medicine and treatments as prescribed by healthcare providers.
  • Monitoring the physical, mental, and emotional conditions of clients and reporting changes to designated parties.
  • Performing personal hygiene care and housekeeping duties.
  • Preparing meals and assisting clients with eating and drinking.
  • Assisting with the use of incontinence products or providing bathroom visit support.
  • Running errands, including shopping for groceries and basic household items.
  • Assisting clients from their beds into chairs or wheelchairs, and back.
  • Accompanying clients on trips, such as medical appointments.
  • Maintaining a clean, tidy, and safe environment.

Direct Care Worker Requirements:

  • High School Diploma or GED.
  • Exceptional ability to assist clients with mobility, personal care, and hygiene needs.
  • Ability to perform basic housekeeping duties, run errands, and prepare meals.
  • Ability to maintain a clean and sterile environment, as well as safely discarding biological waste.
  • Excellent interpersonal skills, a caring approach, and physically fit.
  • Willingness to work irregular hours, including evening hours, weekends, and holidays.
